requisition line in cart automatically added to old, closed requisition

Summary:

I have a user that is creating a requisition line and after viewing it in her cart, it is automatically added to an old req that has been closed for 3 years. I cannot figure out why or how to resolve it.

When the user creates a new req line and views it in her cart, it is automatically added to an old existing requisition that has been closed for 3 years. There's nothing that I can see that links them - it's a different supplier, no source agreement, etc.
