Unable to add responses to Potential Assessment Questionnaire

Summary:

We have added the standard questions into the potential assessment, but we are unable to add the responses. The Potential Assessment Rating model is already configured. We also tried creating new questions, but the response header within the question does not show all the fields required. Tried this on multiple browsers as well.
