Query to get a data dump for a Benefit plan

Summary:

We have a requirement to identify last updated by name and time stamp for Beneficiary related information on Life insurance plans. This is for legal reason we wanted to share that employee has updated the Beneficiary percentages with date and time stamp on it. We basically are looking for a data dump of any changes made on a Benefit plan. Please let me know if you have any query that would pull this information.
