Why is 'Questionnaires' page filling up with activity?

We do not have multiple Questionnaires configured, yet, activity by HR Admins posting jobs and Candidates answering questions seems to be filling up as Questionnaires. What is causing this and should we turn something off so that this doesn't occur? We now have hundreds of Questionnaires with all the activity over the last year(s).
