Create Payment document form keeps scrolling

Summary:

Attempting to Create a new Payment Document From Setup and Maint. | Cash Management | Manage Bank Accounts. Select to edit one of the accounts, scroll to the bottom and select to + or Action, Create Payment Document. The new form pops open however continuously scrolls to the right and I am unable to add any entries into the form.
