Create a custom phone type with all 3 fields including phone extension

The client needs a field similar to that of the "WORK" phone that contains all the fields like the phone extension. We created an additional phone type but it doesn't seem to include the extension field. Can something be done about this? Is there a configuration to accomplish this?
