Relationship Control Interview Extension

We are attempting to build an interview extension for a relationship control.

See attached example rulebase. It contains commentary and hopefully is self-expanatory.

The relationship control select list has no options.

Is it possible to build a Interview Extension relationship control? If so, what are we doing wrong?

Rulebase is August 17. Upgrading to 18A does not resolve.
