Query to Find Element Entry

Summary:

We are looking to develop a report that can help us replicate element entry screen for Overtime Hours as shown in the screen below. I have also included the code that we are using to retrieve Pay_Value for the over time. However this doesn't provide us the hours worked or the original date earned. Original date earned is available in the screenshot provided. Can someone please help to advise the correct column name for the Original date earned?

Also Overtime hours are in the previous months, does someone has the code snippet to include the hours into the report?
