Learning Assignment Status Automatically changed to "Completed" without intervention

Good day,

I would like to know if you experience this kind of issue, Currently we have an issue with 1 of employee where her learning assignment is automatically marked as "Completed" here is the scenario:

1st Scenario: Learner have an assignment on offering for April 08, 2024, the activity is marked by admin as "Not passed"
2nd Scenario: Learner is enrolled again into the same course but in different offering, but this this time, the assignment is automatically mark as "Completed" without having intervention from the admins, it also no sign of activity progress and the completion date is 04/12/2024 which is way before the actual activity date 04/17/2024.
