Query to fetch the individual Manager Rating and Employee rating corresponding to each Goal.

Summary:

I am creating a custom report to get the employee and manager ratings for each performance goal. I got the summary ratings from table 'HRA_EVAL_RATINGS'. But for each item it is not there in the table.

Please help me to get this out.
