Interview Feedback Notification Blank

When an interview feedback form is sent to a Hiring Manager to complete, it is showing as blank and they are not able to get into the form to complete it.

We do have a custom Employee role - however for testing I have removed this and added the OTB role and the notification is still blank.

Has anyone experienced this before and how do we resolve it?
