Employee not able to add performance goals because performance goals section not showing

I have an employee with the following issue. when he tries to add his performance goals performance goals section isn't showing and the review period field is empty as shown in the below screenshot:

employee is eligible for the goal plan and he has a performance document. why is this happening and how can i fix this issue ASAP?
