Document - Add nomination forms

we have a client request, where we need to add the nomination forms for individual compensation for a particular reward. Can we do it through the document of records? Also is there any way to add it to the ICP plan and make it editable for the requestor who is raising the reward request. There are different nomination forms for different business units.
