Counting Unsubscribes from custom unsubscribe form

Hi,

Currently, we don't use the default unsubscribe form but use a custom-made unsubscribe landing page & form.
We understand using a custom unsubscribe landing page & form has a downside in reporting: in Campaign Analysis Overview reports on Eloqua Insight we cannot see the right number of unsubscribes in the "Total Unsubscribes" column. Instead, unsubscribes can be counted in the "Form Submissions" column.

So we're now considering of using the default unsubscribe form, but I have two questions here.

1. Where can I find the default unsubscribe landing page and form?
2. If we use the default unsubscribe form, unsubscribes from it will still be include in "Total Form Submissions"?
