Why are the Recruiter and Hiring Manager still able to raise requisitions for a position which either already has an approved offer or is already filled?

Hi All,
The Recruiter and Hiring Manager are still able to raise requisitions for a position which either already has a candidate in Offer-Approved state or is already filled.
Because of this multiple requisitions for the same position are getting created by different people (recruiters/ Hiring managers).
What shall we do to stop this issue?
