Why aren’t the newest versions of prescreening questions being loaded on new requisitions?

Summary:

When a user creates a new requisition via the "Existing Requisition" method, the old versions of the prescreening and disqualification questions are being added to the Questionnaire. This is not the desired functionality - the newer versions of the questions are updated with correct wording and conditions. around 250 active requisitions are impacted.

It seems to me that ORC is trying to recreate the questionnaires from the old requisition exactly how they were (containing inactive questions and old versions). However, those questionnaires are outdated. This results in questions that have incorrect wording, conditions aren't right, and required disqualifications are missing. We've also discovered several requisitions that are completely broken (a missing disqualification question or response-dependency prevents applications from being submitted all together).
