Form submit action gives error for one form

Hello!

I have been having a status - status "error" set by app when using the form submit action step in Eloqua Prod. I tried to deleted cookies and cache and reprocess the contact. I swapped the action steps with the new one and the result is the same - the same error status.

My form has 15 fields only; we are not reaching any limit.

I would appreciate any suggestions & help on this error.
