Requirement to capture additional info during phone screening phase

Summary:

We have a requirement to capture additional details from Candidates during Phone screen interview. We would like the questions to be available on the candidate application as Recruiter facing question, so the recruiter can go and capture the responses from the candidates on the Job application during the phone screening interview.

We have explored the options of capturing them as Interview Feedback question or Extra Info on candidate application, but both these options has some downsides to them.

With Interview Feedback questions, Recruiter needs assign the Feedback request to themself and also they can only access the Submit Feedback from notification. So, it will be difficult and time consuming for the recruiters to identify the notifications for the correct candidate and submit the feedback.
