In which table exactly are the guided journey questionnaire responses getting stored?

PER_CHK_GUIDED_RESPONSES
We have added a guided journey in the employee referral (Submit a Candidate) page, and have added a questionnaire task. We require the inputs submitted by the employee in the questionnaire task (while submitting a candidate) from the guided journey, to display in a report. Also, we require this data to be fetched specifically against each candidate.
