After selecting the 'Employee Only' option, a 'pending action item' message is popping up

After selecting the employee only option in plan, there is an message popping up like 'pending action item'. I have opened and checked in the pending actions, it is showing like designate the dependent for employee only option. I assume that employee only option configuration is missed something, Please find attached option configuration screen shots for the reference and action message in the below. Can you please correct me with my configuration if I'm wrong.
