OCI: Server Fails to Boot due to File System Corruption

Symptoms

Due to block volume file system corruption, access to the instance fails and the server fails to come up after reboot.

Console error:

Started LVM2 PV scan on device 202:33.
systemd-fsck[1419]: /dev/xvdh contains a file system with errors, check forced.
systemd-fsck[1421]: DBORA: clean, 5176/1310720 files, 1622523/5242880 blocks
systemd-fsck[1420]: /dev/xvdb: clean, 36188/3276800 files, 3005135/13107200 blocks
systemd-fsck[1422]: BOOT: clean, 333/32768 files, 20937/131072 blocks
Started File System Check on /dev/xvdb.
Started File System Check on /dev/disk/by-label/DBORA.

Cause

Due to high orphaned inode list on block volume, file system was corrupted and ssh access also fails to the instance.
