Document link not showing up on Document Type Journey Task, any advice?

Summary:

We have configured a journey task of type Document to show a document.
For some reason the Link to the Document or Preview the document is NOT showing. Instead it shows blank space (2nd screenshot)
Please advise any idea what could be missing
