how display member name alias on a form

I am having trouble displaying the member name and the alias side by side in the rows of this form. What is shown below is the alias. The member names are the actual account numbers (e.g. 6000, 7000, etc…). So i want to see 6000 : Internal Direct Costs, etc…. I know I can set my user preferences to display member name : Alias but that is not how i want to do this because that then displays the member names everywhere and most of our member names are not helpful.
