Courses showing unreconciled changes, when no changes have been made

We keep coming across courses in our catalogue which show that learners are missing the most recent changes. But we know that no one in the team has made any changes.

I've just gone into one of our mandatory courses and it is showing there are 3112 learners missing the latest changes. However, no changes have been made to this course. It isn't even the full number of learners enrolled onto the training that are missing the change. We do have a learning initiative set up, but we only have a few new people enrolled each week. I specifically checked all our mandatory learning a few weeks ago, so I know that there were no learners missing changes at that point.
