RTF template to enable the checkbox option in generated email

Summary:

I have a requirement like need to send mail to Manager asking to wheather contract extension is planned or not with No and Yes checkbox. ?

Content (please ensure you mask any confidential information):

Please confirm if itdoable with RTF template or not If manager wants to extend the contract then hewill select Yes checkbox and mention the extension date. If not to extend ,then simple select No checkbox and reply to generated /received mail.
