How to add consent cookie in careers page as checkboxes

Summary:

We want to add the following statements as checkboxes on clicking apply now on careers page(after where they enter their email id):

I would like to receive marketing communication on products, services, or events.
I provide consent to process my personal data in line with the Privacy Notice*.
 

Ensure that the 2nd checkbox is kept as mandatory and the Privacy Notice includes a hyperlink that directs users to the Privacy Notice page of our company. Additionally, the submit button should remain disabled until all required fields are correctly filled out and 2nd checkbox is clicked.
