OCI: Unable to Boot Instance, Getting Error "can't boot blockdev"

Oracle Cloud Infrastructure - Version N/A and later
Linux x86-64 on Oracle Public Cloud

Symptoms:

Instance is not booting up and getting into emergency mode with the following error message below.

Reloading system manager configuration
Starting default target
[ 320.851780] /dev/sdb1: Can't open blockdev
You are in emergency mode. After logging in, type "journalctl -xb" to view
system logs, "systemctl reboot" to reboot, "systemctl default" or "exit"
to boot into default mode.

Cannot open access to console, the root account is locked.
See sulogin(8) man page for more details.

Cause:

Duplicate entry from the newly added block storage /dev/sdb.
