Redwood Termination Page Has Extra Fields and Missing "Notice Period Worked"

Hi everyone!
In the Redwood HR/Hiring Manager Termination "When and Why" Page, three fields appear that are not in Responsive:

  1. What are you Termininating?
  2. Why are you making changes to direct reports?
  3. What's the action name for direct report changes?

Help me to replicate this issue? Not able to find in VBS Business rule to get this field!
