Reminder Alert for ILT/Blended Offering

Summary:

I know this topic has come up several times before, however, everything I tried from those posts are not working.

I imported the ILT reminder alert from the post shared with the other custom alerts and it errored out. I have read through all of the available posts/threads on filters for an ILT/Blended Reminder alert and they have all ended in the alert erroring or warning. I want it to be sent 5 days before the activity date. In the expression below I have it set to 1 day, for testing purposes.

What am I missing or what should I change? These are the filters I am using -
