Worker self evaluation header

Summary:

We see a line on the performance documents saying "Review and evaluate each topic in your performance document" and it needs to be removed, I have checked everywhere in the configurations that I could think of but couldn't find anything related to it, Any help to find it is greatly appreciated. :)
