Document Type after Redwood

Our organization updated to Redwood this past August. Since then, a user can't access a certain Document Type. Steps to recreate issue:

  1. in My Client Groups, user goes to Document Records
  2. user clicks on an employee
  3. user clicks Add
  4. the Document Type appears in list, and user clicks it
  5. fields don't appear

Before the Redwood update, the fields did appear. But now, they don't. Nothing else was changed. Where should I check to fix this?
