Duplicate notifications received for goals assigned

Summary:

When goal is being assigned to worker by manager or HR, worker is receiving duplicate notifications. They are receiving same email, one with subject as setup and one with same subject with prefix FYI. Also, we tried to disable mail from alerts setup and just enabled worklist alert for same, so along with bell icon worker is also receiving FYI email.

Ideally, if worklist emails have been setup on alert page, worker should only get bell icon notifications. Why are they receiving FYI mail for same?
