Credit Management and we cannot make a Case Folder reach a status Rejected

When a Case Folder is created from a sales order that fails credit check, the folder opens with status NEW.
After assigning myself as Credit Analyst and adding a valid recommendation (for example: Approve Source Transaction Credit Request or Decline Source Transaction Credit Request), the status changes to Analyst Working, but the system does not allow me to submit the case folder into an approval flow.

Symptoms

• The Actions menu never shows Submit for Approval.
• Reject is always disabled/greyed out.
• Only Approve appears, but even after saving a valid recommendation, the status stays Analyst Working.
• No case folders in the environment ever reach
