Checklist Task- report path not rendering specific employee's answers

Summary:

I created a report DM and added the parameters- see screenshot below. When LM opens the task in journeys and clicks to view the report with employee's reponses; they do not see that specific employee's data associated with the task. They see one of the employee's data but not the one associated with the assigned task.
