Cloud Form Submit Error in Program Canvas

Cloud Form Submit configuration can be saved successfully, but an error is prompted afterwards.

We have the Cloud Form Submit App installed; we are able to configure the Cloud Form Submit action item in program canvases and save our changes successfully.

However, we then receive an error message at the program canvas level stating that we need to configure the steps.

In addition, I checked a few existing programs where we implemented Cloud Form Submits and all requests are pending with the status "App notified successfully - Awaiting app to process".
