Acceptance Rate Calculation

Hello
I would like to know how other companies are calculating Acceptance Rate. This is the calculation we currently use:
=[# Offers Accepted]/([# Offers Accepted]+[# Offers Declined] + [# Offers Reneged] + [# Offers Rescinded])
Also, are you counting all offers created?
For example,
If a candidate applies to a requisition and is given an offer and refuses the offer, then another offer is presented to the same candidate and refuses the offer again and then a 3rd offer is presented to the same candidate and accepts the offer. Do you count that has 3 separate offers in the Acceptance Rate calculation or do you only count the latest job offer to each candidate?
