Form CC-305 Formatting issue on Job Application Page in ORC

We are experiencing a formatting issue of the Form CC-305 on US Job Applications within our career site. We have created an out of the box career site with no custom CSS or JavaScript. This is an issue in all pods. Has anyone had this issue in the past? The form in the Employee Self Service page is formatted correctly.

Oracle is claiming a potential HTML issue, but we are unsure of where it would be coming from. We have no customization on the career site at all at the moment.
