How to resolve if Employee has two Federal Forms?

Hello Experts,

We have an employee that has two federal forms on file - one is signed and the other is unsigned. We need to identify why this occurred, determine the appropriate resolution and establish steps to prevent it from happening again.

Tried to delete also the other form but encountered an error.
