Need to help to prepare the SO FBDI for subscription and coverage(Care plan) items

in Order Management

Summary:

Could you please help me to prepare the FBDI template to add subscription item and coverage items under the document reference tab.

i tried with below details but it didn't worked and getting this error.

if anyone have the prepare the template handy please share it here that would helps a lot.
