Element Overlap

We noticed 10/05/2025 2 salary change life events were triggered 1 is in processed status, 2nd one is in manually added. to clear this event, we unprocessed open event and unprocessed salary change event and processed again. then tried to add/process open event and we faced the below issue.
we checked the element entries and found few entries already exists with source as Element Entry page, few others are benefits.
is this employee record messed up by application or some one loaded these element entries via HDL or so?

Error- There was problem in saving elections because another enrollment exists in the same element that this enrollment uses. (BEN-991905)
