Supplier profile change request gets canceled after approval

Summary:
Hi Team,

One of our customers is trying to create a profile change request to add bank accounts and address details,
they are able to submit a change request but when they try to approve it the request is getting cancelled and it is showing an error IBY-IBY_0000
Cancellation reason - 'Process to apply the approved change request to the supplier profile is failed due to unexpected errors'

We would appreciate your guidance on how to analyze and resolve this issue
