Loading Extensible Flexfields

Summary:

We are having trouble loading a new extensible Flex Field Job level to our jobs. I have made sure the Flex Field is deployed and you can manually add it to the job. You can see the fields I have in my template but they are failing. Are there other columns missing from the spreadsheet that are required to import the Extensible Flexfield?
