Edit Pending Worker Error: A database constraint was violated while storing the changes in database

Hello! When completing an Edit Pending Worker Transaction in Redwood, we have encountered the error message: A database constraint was violated while storing the changes in database when trying to submit. The only thing we noticed that was different is our "action reason" was already pre-populated which generally isn't pre-populated.

Does anyone know what could be causing this error message? It is our first time coming across it.
