Employee Submit ICP and approval show submiited ICP and deleted ICP from another plan

Summary:

Hello,
Employee submitted the ICP and approval show the subitted ICP from [Plan A] & also Deleted ICP form another plan [Plan B]

No Approval History shown on approval notification and on the transaction console side, even though the transaction still in progress

Any input regarding this? why deleted ICP shown with the new one?
