I need a BIP/OTBI report If the rating in any of the question is 3 & below-BHR intervene & solve.

Proposing to integrate three feedback forms into our HRMS, triggered automatically at the 30-, 60-, and 90-day milestones, to capture each new joiner’s onboarding experience in real time. 

5-scale rating with 9 questions along with 1 open-ended question 

If the rating in any of the question is 3 & below – we can then ask BHRs to intervene & resolve the query with a TAT of 7 days.

please suggest how to achieve this requirement. by through BIP or OTBI Part.

And Journey part- I created Onboarding Journey. Create Questionnaire in HR Checklist subscriber. it is good or change Please suggest.
