Few of existing questionnaire is suddenly disappear from LOV for task creation task library 26B

Hi,

i am facing a issue - few of the questionnaire disappeared in LOV in the existing task or when i am trying to create a new task, not able to find them - from Task library. Also tried to create a new questionnaire but no luck, it's NOT showing in LOV. 26B. Subscriber is HR checklist. i have many HR checklist questionnaire, few of them disappeared and newly created ones are not appearing.

Any idea how to fix this?
