Assessment Marked as Failed Despite Selecting Correct Answers

Hi Community,

We are currently on Oracle Fusion HCM Learning, Release 26B (Redwood UI) and facing an issue with assessments embedded in course offerings.

Issue:
Learners select the correct answers during an assessment, however upon submission the assessment is being marked as Failed. The passing/mastery score is configured correctly on the content item. Mastery score is set correctly on the assessment content item E.G 80 as per customer requirement

Ask:
Has anyone encountered this issue and found a resolution or workaround? Is there a known bug/patch related to this in 26B?

Any SR numbers or Customer Connect threads would also be appreciated.
