HireRight ERROR: integration preventing order from being accepted by HireRight

Hi all. We started experiencing an issue yesterday where we are receiving error notification emails from HireRight. We are looking to set up a meeting with HireRight but in the meantime I wanted to share the error here in case anyone has received something similar and has a suggestion on anything to look at in Oracle. Below is the email from HireRight that came back:
