value set for an LOV in an EFF, specifically for the Patient field when creating a Benefits Claim?

We have a requirement to display both the employee's name and their registered contacts (dependents/beneficiaries) in the Patient LOV when creating a benefits claim. Any references or existing configurations would be greatly appreciated.
