Request feedback form not triggered to interviewers

Summary:

Dear Experts,

When i schedule an interview, there is an option to select interview feedback and add the feedback questionnaire.

However once the interview is completed, the interview does not receive the "Feedback form".

I have checked IRC_FEEDBACK_REQUEST in alert composer, and the alert simulate is set to "No".

Still the notification is not triggered. Can someone help with the solution.
